Requests for Emergency Reviewers

We are now requiring the completion and submission of standardized forms for situations where the TARGET Administrators are being asked to establish emergency Proxy or Alternate reviewers.  We are doing this because a standardized form makes the process clearer and easier to follow, and it makes everybody follow the same rules.  This makes it easier to audit to ensure that no changes are made without proper authorization.  The forms require Adobe Acrobat Reader to view, and can be filled out on the computer, except for the authorizing signature.  The completed form(s) should be printed and faxed (5-2336) or hand-carried to Rachel McGathy, ADMN 316.

Emergency Proxy Request Form

This form requests the establishment of one or more emergency proxy reviewers.  These persons will be able to review immaterial transactions only.  The form must be signed and authorized by someone of responsibility in the department.  Examples are: Dean, Director, Department Head, Assistant Deans/Directors, etc.  If no such person is available in your department, please move "up" a level, such as to the Dean's Office.  There are no restrictions on who can be selected to assume emergency proxy status.

Emergency Alternate Request Form

This form requests the establishment of an emergency alternate reviewer.  This person will be able to review material transactions only.  The form must be signed and authorized by someone of responsibility in the department.  Examples are: Dean, Director, Department Head, Assistant Deans/Directors, etc.  If no such person is available in your department, please move "up" a level, such as to the Dean's Office.  The emergency alternate reviewer must also be a person of responsibility in the department, and his/her position must belong in the same budgetary unit as either the primary or normal alternate reviewer.  No support staff may assume alternate reviewer status.
