CAPS Outreach Services

To request a presentation, use your browser to print the request form. Fill out the form and mail it to the address below. To request a presentation by phone, please dial 575-5276. Be prepared to provide the same information requested on the request form.

CAPS will make every effort to meet your request but may not be able to do so. Please allow no fewer than two (2) weeks notice for presentations. We require a minimum of ten (10) participants for a presentation.

Scheduling is flexible within the availability of CAPS clinicians. It is unusual for CAPS to do a presentation on a weekend or after 8:00 p.m on weeknights. The Outreach Coordinator is glad to work with you to find a time that is mutually convenient.
