*Note: When forage is to be planted, select the establishment code to match the desired forage, whether for hay or pasture, and then select the corresponding production or maintenance codes for that forage for hay, pasture, or both hay and pasture if requested. If no forage will be planted and production of existing forage is desired, select only the hay production or pasture maintenance codes for that forage. On new cleared land use fallow code # 19 for previous crop on the soil test form.
