Ascend ID and Security Request
Information Services provides Ascend ID's to associates on the Fayetteville campus of the University of Arkansas with approved security. To request an ID, please complete a "Request for Ascend ID Form" and sign.
This form must be approved by your dean, department head, or the account signatory. In addition, anyone requesting access to donor records MUST sign the statement of confidentiality.
Users should then return the form to:
