Information on Interactive
Forms
The University of Arkansas requires a taxpayer identification number
certification from all vendors. In order to
comply with this policy, providers are required to submit a valid Taxpayer
Identification Number (TIN). A
TIN is either a 9 digit Social Security Number or a 9 digit Employer
Identification Number (EIN).
In order to comply with this policy, departments are asked to request a W9 form from new service providers (vendors) at earliest convenience to avoid delay in payments. This request also applies to new employees only for reimbursable moving expenses.
After filling out the form print it out and sign it and return to:

2. After the download is complete, quit your Web browser.
3. Double-click the newly downloaded file
4. Follow the instructions on your screen.
If there is a failure
at any point during the installation of Acrobat Reader, the installer performs
a
complete un-install.
For this reason, it is important not to close the installer application
by clicking its
close box in the upper
right corner of the background window after clicking the Thank You"
dialog
box that appears
at the end of the installation. If you wait for a second or two, the installer
will
automatically close
the background windows after the installation is complete.
Information on Interactive forms
An Interactive PDF is a document created for on-line use.
In addition to view and print functionality,
you can use Adobe Acrobat Reader to actually fill in the form,
print it out and mail it to it's final
destination. If you have the full version of Adobe Acrobat,
you can save your forms to a disk. Note: In
order to save the form AND the information you entered, you must
have the full retail version
of Adobe Acrobat. Adobe Acrobat is not free software, however
the Reader is completely free
and available for download here.

Here's How: While
you are viewing the Interactive PDF in Acrobat, simply click on the Hand
Tool in
the Acrobat Reader Toolbox, then
move your mouse to the desired field on the form and click. It's
that
simple! Once you are editing
the form you can use the 'TAB' key to move to the next field. For
forms
that require an authentic signature,
simply fill in the form on-line, print it out, and sign. No
more typewriter hassles!
Note: These forms work best with Adobe Acrobat 4.0 and above.
