The convention will be held on Tuesday, April 6 and Wednesday, April 7, 2010 at the Holiday Inn Convention Center and Hotel, Springdale, AR.

After the form is submitted, an itemized invoice will sent via email. This form may be printed.  Mail your check to the above address with your printed form or invoice.

Deadline for preregistration is March 1, 2010. Registration is $50 after preregistration deadline.

Please indicate tickets needed for each person. The registration fee covers all convention events except meal functions. Spouse's registration is complimentary. Tickets for meals are required for each person and are available at prices indicated. Only a limited number of meal tickets will be available for on-site registrants. Submit multiple registration forms if needed.

Registration and Cancellation Policy: Registration is required for all events. Registration can be cancelled by written request to OFPA. No refunds after March 1. Substitutions are allowed. “No shows” will not receive a refund.
