31st Annual 18-Hole Golf Tournament
Shadow Valley Country Club
Tuesday April 6, 2010, 9:00 am Tee Time

The golf tournament will be a 4-Man Scramble. Prizes will be awarded in two flights determined by score. Entry fee includes range balls, ½ cart, greens fees and lunch. Other features include a Long Drive Contest, Closest to Pin, and a Putting Contest. To register, complete and submit this form. Once the form is submitted, an itemized invoice will sent via email. Mail your check to the above address with your printed form or invoice.

Cost: $110 per person or $440 per team. Proceeds from the tournament support OFPA scholarships.  Pre-registration deadline is March 1, 2010.

Deadline for preregistration is March 1, 2010. Registration is an additional $25 after March 1.
Golf tournament registration can be cancelled by written request to OFPA. No refunds after March 1, 2010. Substitutions are allowed. “No-shows” will not receive a refund.
