34th Annual 18-Hole Golf Tournament
Shadow Valley Country Club
Tuesday April 2, 2013, 9:00 am Tee Time

The golf tournament will be a 4-Man Scramble. Prizes will be awarded in two flights determined by score. Entry fee includes range balls, ½ cart, greens fees and lunch. Other features include a Long Drive Contest, Closest to Pin, and a Putting Contest. Appropriate golf attire required (collared shirts and no jeans).  You must pay for your own club rental.  To register, complete and submit this form. Once the form is submitted, an itemized invoice will be sent via email. Mail your check to the above address with your printed form or invoice. Payment can also be made by credit card from our website. 

Cost: $110 per person or $440 per team. Tournament sponsorships and hole sponsorships are available. 

Proceeds from the tournament support the Justin R. Morris and Jerry Dickson scholarships awarded by OFPA.

Deadline for preregistration is March 25, 2013. Registration is an additional $25 after March 25.
Golf tournament registration can be cancelled by written request to OFPA. No refunds after March 25, 2013. Substitutions are allowed. “No-shows” will not receive a refund.
